Define the decision first
Before writing questions, decide what the team needs to learn and what it could change. A focused survey is easier to complete and easier to act on.
Ask less, learn more
Start with the smallest question set that can identify a useful signal. Remove fields that are interesting but will not change a decision.
Choose the channel that fits
Use a QR code for a physical moment, an in-product prompt for a digital workflow, or an opted-in message when a later follow-up makes sense.
Tell people what happens next
A short explanation of why you are asking and who will see the response can make the request feel more trustworthy.
Close the loop
Thank respondents, route urgent feedback, and share changes when appropriate. The response experience is part of the survey experience.
